Title NEEDLE HOLDER
Abstract PROBLEM TO BE SOLVED: To provide a needle holder capable of easily retaining and releasing a thread without any complicated operation and, even if the needle holder has just a small swing angle, easily and quickly executing a mechanical ligature irrespective of the inside/outside the body.
SOLUTION: This needle holder 1 is provided with a long needle holder body 11, a grip section 12 disposed with a gripping member having a rotational gripping member 12A and a fixed gripping member 12B at its distal end side and freely opened/closed by rotationally journaling the rotational gripping member, and an operation section 13 disposed at the proximal side of the needle holder body and capable of opening/closing the gripping part. The proximal end of the rotational gripping member constituting the gripping section is provided with a thread storage section 2, the thread storage section is opened to store the thread when the gripping part is closed, while the opening of the thread storage section is contracted by the rotational turning of the rotational gripping member when the gripping section is opened, and the stored thread is retained in a thread retaining space X without being detached.

従来の腹腔鏡下手術等をはじめとする手術に用いる持針器は、一般的に、長尺な持針器本体と、持針器本体の先端側に縫合糸や針付糸等を把持する開閉自在な把持部を具備し、持針器本体の基端側（術者の手元側）に把持部を開閉操作する、たとえば、ハンドル状の操作部を具備した構成となっている。



また、たとえば、以上のような構成を有するとともに、持針器本体の外周面上に、糸や針付糸を固定するための引掛片等からなる糸固定手段が設けられている持針器が提案されている（特許文献1）。



そして、これら従来の持針器を用いた器械結紮は、図4に例示したように、たとえば、2つの持針器A，Bを用意し、一方の持針器Aで、他方の持針器Bの本体（軸体）に縫合糸や針付糸等の糸を巻き付ける。ついで、持針器Aで糸の一端（Long Tail）を把持して糸を引き抜くとともに、糸を巻き付けられた持針器Bで糸の他端（Short Tail）を把持して糸を持針器Aによる引き抜き方向とは逆方向に引き抜くことで、器械結紮を行っている。

【請求項1】
 
長尺な持針器本体と、持針器本体の先端に開閉自在な把持部と、持針器本体の基端に操作部とを有し、前記把持部は、固定把持部材と回動把持部材とからなるとともに、回動把持部材は、持針器本体と軸支回動可能に連結されており、前記操作部は、回動把持部材の回動による把持部の開閉操作が可能とされている持針器において、回動把持部材は、持針器本体の短尺方向に、把持部が閉状態において上方に開口した溝状の糸収容部を有し、この糸収容部は、把持部が閉状態において糸が収容可能であるとともに、把持部を開状態とする回動把持部材の回動によって縮小または閉鎖可能とされていることで、収容された糸が脱離せずに糸収容部に保持されることを特徴とする持針器。

【請求項2】
 
持針器本体の内部には、操作部による操作力を回動把持部材に伝達するための操作力伝達中芯部材が摺動自在に装入され、この操作力伝達中芯部材の先端側を回動把持部材と連結させるとともに、基端側を操作部と連結することで、回動把持部材を軸支回動可能としている請求項1記載の持針器。
