| Abstract |
PROBLEM TO BE SOLVED: To provide a compact and horizontal type nasal CPAP device designed to alleviate strain on premature babies during treatment with it applied to them. SOLUTION: The nasal CPAP device comprises an air supply nozzle 2 to discharge an air jet flow, a passage enlarging part 4 connected with an outlet of the air supply nozzle 2, a nasal cavity attachment 6 connected with the passage enlarging part 4 perpendicular to the air jet flow, a jet flow diverging plate 8 provided on an opposite side of the nasal cavity attachment 6 to an center of the air jet flow with offset in the passage enlarging part 4, and an expiratory exhaust pipe 10 connected with the passage enlarging part 4 on the opposite side of the nasal cavity attachment 6 to the jet flow diverging plate 8. |

(In Japanese) 近年、非侵襲的な呼吸管理という観点から、ナーザルCPAP（Continuous Positive Airway Pressure）が未熟児の呼吸障害の治療に用いられつつある。CPAPは、呼気相終末においても気道内圧を陽圧に保ち、呼気相での肺胞虚脱を防止することにより機能的残気量を増加させ、肺内シャントを減少し、低酸素血症を改善する。鼻孔に陽圧を加えるものをナーザルCPAPと呼ぶ。 【0003】 ナーザルCPAPの利点は、（1）気管内挿管する必要がなく簡易にCPAPがかけられる、（2）挿管チューブの気道抵抗がなく呼吸仕事量が軽減できる、（3）気管内挿管管理に伴う気道感染症を回避することができることなどが挙げられる。 【0004】 しかしながら他方においては、（1）鼻孔取り付け部の鼻孔への強い圧迫による鼻孔狭窄、鼻中隔壊死を発症する可能性がある、（2）固定方法が難しく、リークにより十分なCPAPを維持することが困難である、（3）食道へも陽圧がかかるため、腸管が拡張して栄養が入れにくくなることがあるといった欠点も存在した。 【0005】 このような欠点を解消するために、微細な自発呼吸にも敏感に反応し、安定した気道内圧を提供し、呼気時の回路内抵抗を解消するため、流体力学的原理を応用したものが登場してきている。例えば、インファントフローシステムと呼ばれるものがある。 |
